GROUND BURIAL (36 hours’ notice)

The use of the Chapel Mausoleum is available at no extra charge. Pall bearers must remain after the committal service to assist in transporting the casket to the grave site. Cemetery staff will not carry the casket.
MAUSOLEUM ENTOMBMENT AND INURNMENT

POLICIES
| Weekday arrivals before 9:00 a.m. and after 3:00 p.m. | $350.00 |

- A solid outside container is required for all casket burials.
- Vault for cremains not required.
THE FOLLOWING ITEMS ARE REQUIRED AT THE TIME OF COMMITTAL:
- Completed cemetery interment order form signed by family.
- State required burial / entombment permit.
- Full payment for services ordered.
- Lebanon Church Cemetery requires a casket enclosure for all disentombments and disinterments before entombment in the Lebanon Chapel Mausoleum. The casket enclosure must be “EnsureASeal”. Contact the cemetery office for details.
- No services are accepted on Sundays, New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day or Christmas
PURCHASE OF GRAVES, CRYPTS & NICHES:
- All graves – $975.00 each.
- Crypt – start at $3,449.00 Niche – start at $1,145.00
COST OF FOUNDATIONS: $1.05/Sq. Inch
DEED TRANSFER FEE: $125.00
RIGHT TO SECOND & THIRD BURIAL: $350.00/EACH – Cremation only
MONUMENT LAYOUT: $125.00/EACH
FOUNDATION DEPTH & CONCRETE INSPECTION: $100.00/Foundation
